Senior Programs

The West Virginia Bureau of Senior Services is a cabinet-level agency within state government and acts as the lead entity for programs serving older West Virginians.  The Bureau along with Metro Area Agency on Aging administers funding to Coalfield Community Action Partnership, Inc. to provide services to our senior citizens in Mingo County.  Coalfield Community Action Partnership Inc. takes great pride in our senior services because we are assisting one of our county’s most valuable resources, our senior citizens.

Senior Centers

We currently have four centers located in Williamson, Delbarton, Gilbert, and Matewan areas. Our senior center sites provide participants with an opportunity to socialize with one another and enjoy a hot nutritious meal.  Our centers also offer exercise equipment, a dining area, computers and a recreation area for seniors to access.  In our centers we also offer many services to our seniors such as assisted transportation, telephoning, trainings, and health screenings.  Our senior center sites are open Monday through Friday 8 am until 2 pm.  The only eligibility for participating in our services is that the individual must be 60 years of age or older.

Home Delivered Meal Program

The Older Americans Act Title III Home Delivered Meals Program is designed to provide meal service to those who are homebound and unable to cook. Coalfield C.A.P., Inc. has home delivered services in the Williamson, Matewan, and Kermit areas. Not only does the home delivered client receive a hot nutritious meal but they are also contacted throughout the week with our telephoning service.  Our service hours for delivery are between the hours of 11:30 a.m. until 2 p.m.

Eligibility Criteria for Home Delivery

Individuals may be able to receive a home delivered meal based on the following:

1. The person is 60 years of age or older, AND is

2. Homebound: The person cannot leave his/her house under normal circumstances (and is therefore unable to participate in the senior center) due to illness, including a terminal illness; incapacitating disability; isolation; and /or lack of transportation.

3. Lives alone and is physically or mentally unable to obtain food and prepare meals, and there is no one else available to obtain food and prepare meals.

In-Home Care Programs

Coalfield Community Action Partnership, Inc. has two programs, funded through the West Virginia Bureau of Senior Services, for caregivers:

FAIR provides in-home respite care for individuals with Alzheimer’s disease or another dementia. Criteria to be eligible are to have a diagnosis of Alzheimer’s disease or another dementia and be a resident of Mingo County.  Our services provides up to 16 hours of respite care per week and a regular break for caregivers.  This program also provides stimulation and socialization for the person with Alzheimer's.  A sliding fee scale is based upon household income.

The LIGHTHOUSE Program is designed to assist those seniors who have functional needs in their homes.  Services are provided by a caregiver employed by Coalfield Community Action Partnership, Inc. after an applicant for the program has been determined eligible by the provider’s Registered Nurse.  This program is unique in that it is funded by the State of West Virginia and participants pay based on their monthly income.  Administered by the West Virginia Bureau of Senior Services, services are delivered by a trained caregiver employed by Coalfield Community Action Partnership, Inc. 

A Lighthouse client can receive up to sixty hours of service per month, based on a Client Assessment and resources available. The Lighthouse Program provides support in four areas:

Personal Care: Grooming, Bathing, Dressing, and Toileting

Mobility: Transferring and Walking
Nutrition: Meal Preparation, Eating, and Grocery and/or Pharmacy Shopping.
Environment: Light Housecleaning, Making and Changing the Client’s Bed, and Client’s Laundry.

To participate in the program one must be at least 60 years of age and meet the medical and financial eligibility requirement.

Coalfield Community Action Partnership Inc. also provides additional In-Home Care Services for those who receive WV Medicaid and Veterans Benefits.

The Personal Care Program is a West Virginia Medicaid reimbursement program funded by West Virginia DHHR and the Bureau of Medical Services.  Personal Care services are medically oriented activities or tasks ordered by a physician, implemented according to a Nursing Plan of care and completed under the supervision of a registered nurse.  Generally, anyone who is 18 or older who is Medicaid eligible and has a Pre Screening Application from a physician is eligible for in-home personal care services which include: Personal hygiene, Physical assistance such as helping a client walk or repositions/transferring of a client, help in feeding, light housecleaning, shopping, and payment of bills.

The Aged and Disabled Waiver is an in - home care program.  Homemaker staff comes to your home to assist with your personal needs.  This program is an alternative to nursing home care.  This means that you have the same type of needs as someone in a nursing home but it does not mean that you must go into a nursing home.  When you first apply for services, a Registered Nurse will contact you for an evaluation in your home.  To be eligible, an individual must have needs in at least five areas of activities of daily living.  Activities of  daily living include some of the following: Eating, Bathing, Dressing, Grooming, Mobility, Toileting and other areas of need.  To receive services from the Waiver program, you must first meet the criteria for both medical and financial eligibility. 

The services that you may receive from the Waiver Program include Case Management and Homemaker Services.  Self-directed options are also available. The services are defined below:

Case Management Services: A Case Manager is a licensed professional who assesses and identifies your needs, helps you plan for your care, and advocates for and connects you with services or resources in the community.

Homemaker Services: A Homemaker is a trained care provider who assists you with bathing, dressing, grooming, eating, fixing your meals, or other personal care needs.  These Homemaker Services are supervised by a Registered Nurse.

The Community Care Program offers services to disabled Veterans who have completed their active duty as required by the Department of Veteran Affairs and receiving supplemental benefits.  Community Care services are ordered by a physician and implemented by a nursing plan of care under the supervision of a registered nurse.  Veterans are referred to our agency by the Department of Veteran Affairs in Huntington, WV.  The agency provides In-Home services such as personal hygiene assistance, physical assistance with walking or transferring, assistance with feeding, light housekeeping, shopping, payment of bills, and respite relief for caregivers.

Senior Centers Offer One Stop Shop Services

Coalfield Community Action Partnership has extended its services to involve outreach offices or "one stop shops" in the remote areas of the county.  Our senior centers located in Gilbert, Matewan and Delbarton have extended hours to provide agency services to the people in these communities.   This concept came about so that citizens in these areas could stop by their local community office to apply for CCAP services which include:

• Emergency Utility Assistance
• Emergency Medication Assistance
• Weatherization Services
• Short and Medium Term Rental Assistance
• Security Deposit Assistance
• Information and Referral Services
